Designer/builder recommendation wanted

Hello,

We're looking to refurbish our kitchen/dining room: the usual sort of thing - opening it out at the back, bringing the sink and cooker forward etc' - can anyone recommend a firm or individual who'd be able to a) quote and b) offer some design advice (particularly around the details that you tend not to think of 'til it's too late)?
