Melbourne Grove Medical Centre ( closed permanently?)

Does anyone know,if the doctors surgery on Melbourne has closed permanently? And if so am I right in thinking that it has merged with the Tessa Jowell GP and all patients transferred over?

It's not "closed". It's joined with the Hambleden practise and moved to the Tessa Jowell GP Surgery.

All Patients were moved over (and should have been notified over time).
