Has anyone been on a committee and had to deal with staff disciplinary issues?

I'd recommend the guidance materials published by ACAS. They publish a statutory Code of Practice on Disciplinary and Grievance Procedures in the workplace - http://www.acas.org.uk/index.aspx?articleid=2174

They also publish more detailed guidance with practical examples- The ACAS Guide - http://www.acas.org.uk/index.aspx?articleid=2179

Both documents can assist employers and employees in making sure that they follow correct procedures when dealing with disciplinary matters.
