Stained Glass Entrance: Recommendations Please

Sadly, our original stained glass entrance was either removed or broken and replaced with safety glass. We've figured out the original design and would like to commission someone to rebuild it. Has anyone dealt with a company they'd recommend for building and installing stained glass panels to fit an original Edwardian design?
