Accident in summer school

My child went to a local summer camp and bumped his forehead there. It was quite obvious with the bruise and bump. When I collected him, no one mention it to me but himself.

In the nursery, there is usually procedure, treatment, phone call and slip. Is it usual that the ppl running summer activities don't record accident?

It would be good practice to record and let parents know of any bumps/accidents.
