Does anyone know if you need to fill in a supplementary form when applying as well as the common application form (am assuming you do)

If you are applying for a church place then yes, if not, then no. It was very confusing when I did it last year, some literature in Southwark says you have to but the school office clearly say no.

i second what mumra says - some of the info makes it look like you need to but when you go into the office and ask they tell you that you only need to fill in a supplementary form if you're applying for a church place.

Yes in that case you definitely need to do that form and I suspect it needs to be sent in with your CAF (by this FRIDAY, 29th Jan). But do check this with the school office - I can't remember where I had to send the church form for my daughter!
