A thumb guard worked for my seven year old. She stopped within a week. I think she basically decided she didn't want to be the kind of person who had to wear such a horrible device. We only used it at night as she found the will power not to do it during the day. I wouldn't inflict it on a 19mo though. It really isn't very nice and quite demeaning if they have to wear it in public.
We used mavala no bite, check spelling! Used it on my then 18m old, tasted foul, she stopped immediately! The only trouble was then napping as it soothed her to sleep! My grandma gave me ?1 and each time she saw me suck it she took away a penny, after one day I had 97p and was cured!!! Was 12 at the time!! Good luck!!
