Based on a new study we published today in JAMA, the answer appears to be: only for some. The day that the FDA granted the Pfizer COVID-19 vaccine an emergency use authorization, we launched a national study of vaccine immune responses in immunosuppressed solid organ transplant recipients. Among 436 COVID-na?ve participants who received a first dose of mRNA vaccine, only 17% mounted detectable antibodies to SARS-CoV-2. This is in stark contrast to immunocompetent people who were vaccinated, of whom 100% mounted detectable antibody; that was true for people who had received either the Pfizer-BioNTech or Moderna vaccine. We also found that those taking anti-metabolites, such as mycophenolate or azathioprine, were about five times less likely to develop antibody responses (8.75% detectable antibody in those taking anti-metabolites versus 41.4% in those not taking them). Naturally, we were disappointed to see these findings, as we were hoping to be able to tell our immunosuppressed patients that the vaccines seemed to work well for them. Given this observation, the CDC should update their new guidelines for vaccinated individuals to warn immunosuppressed people that they still may be susceptible to COVID-19 after vaccination. As the CDC guidelines are currently written, they assume that vaccination means immunity. Our study shows that this is unlikely for most transplant recipients, and one could guess that our findings (especially those concerning anti-metabolites) could also apply to other immunosuppressed patients, such as those with autoimmune conditions. Of note, our previous research has not found that immunosuppressed transplant patients are at increased risk of COVID-19 mortality as we thought might be the case. But regardless, the vaccine does not seem to work as well in this same population. As a transplant surgeon, there are a few implications for my patients. First, it seems pretty clear that immunosuppressed individuals need (at least) their second vaccine dose; proposals to stop at one dose until the rest of the population is vaccinated apply to immunocompetent people, but not to my patients. Second, it is critically important for immunosuppressed individuals to realize that they are not necessarily immune after receiving the vaccine, and to talk to their providers about antibody testing before relaxing protective behaviors. Fortunately, semiquantitative antibody tests like the ones used in our study are widely available, and correlate well with neutralizing immunity. Our study is ongoing, and soon we will have data from the second dose, as well as deeper studies of T-cell and B-cell responses, which can confer immunity even when antibodies are not present. We are also studying other vulnerable populations. Enrollment is open, we welcome new participants, and we hope to share more information soon.   10. I was called by the energy people this morning to check that everything was now ok. Apparently there was an explosion at the substation adjacent to 25 Lacon Road. Firefighters came very quickly and contained things, and there was no fire. Some people didn't get their power back till 3am, so Ulverscroft Road was relatively lucky! For future reference (let's hope not needed) the emergency number is 105.

  21. ?9.95?! You should have taken it back and complained. They would have refunded you. I doubt there is much crossover between M&S customers and Co-op customers. Once M&S opened, I rarely used the Co-op. Even though I fully support their ethic, apart from a few things, M&S just seemed better value, and was actually cheaper than I had expected. ETA: However, once I can, I will definitely check the new Co-op out. But they had always seemed to have supply problems, or management problems, or both, particularly in the fruit and veg section. ETA: I think M&S generally have problems, though less so with the food side. They have major problems on the clothing side, for various reasons, and now they are closing their bank. Which is very annoying as I have a current account with them (luckily not my main account) and now I've got to move it elsewhere. Though at least it's much easier to do that these days. Sorry admin, way off topic, stream of consciousness, as you were, back to the Co-op 🙂
  22. I find pigeon behaviour fascinating. A pair of wood pigeons come to feed at the ground feeder in my garden. The male (I presume. Large and bossy 🤣) takes over the feeder, wolfing down seed. Whenever the female (I presume. Smaller and timid). gets anywhere near it, she is chased away. She is left to scavenge for seed in other parts of the garden nearby. No wonder he is so fat and she is so thin! (I have now started putting another pile of seed on the ground a little way from the first one, but it isn't in a feeder). However sometimes another male wood pigeon comes down and then the two males have a fight to see which gets to stay. Also interestingly, all the sparrows seem to come down for their first feed of the day around the same time as the pigeons. Weird. I don't put the seed out at a certain time each day, so it isn't that!

The thing is, like the terrible dangerous (for pedestrians) junction where the South Circular meets the continuation of Lordship Lane by the derelict pub, if someone was killed or seriously injured here, they would come up with a solution PDQ. Speaking of which, I thought it had been agreed to install pedestrian crossings at the South Circular junction. Does anybody have an update on that? I can't remember what the timescale was.
